Were looking for compassionate, responsible, and adaptable Travel RN Case Managers to join Good Samaritans Rapid Response Team who can provide care to assigned patients/clients in a caring, safe, and efficient manner. This position comes with premium compensation starting at $60 / hour, guaranteed full-time hours, and a comprehensive benefits package. On top of that, we cover your costs associated with travel, lodging and food. If you love travel, we offer short-term assignments that vary from 2-4 weeks and employees are given 24 hours notice of their next assignment. The RN Case Manager will be expected to travel 100% of the time on the job. Must be willing and able to work in COVID positive environments and travel to different states across the Good Samaritan Society footprint. The Registered Nurse (RN) Case Manager is responsible for utilizing the nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation) to provide professional nursing services to patients/clients at home. The nursing process includes the documentation of human responses to actual or potential health problems in the home environment. This practice includes dependent, interdependent and independent functions. Collaborates with patient/client and family, other inter-professional colleagues, such as physicians, to plan, implement and evaluate care. Demonstrates competency and practices within the full scope of nursing expertise/knowledge and utilizes appropriate age and population specific standards as designated in their assigned setting. Cares for patients/clients in all phases of preventative care, health maintenance, diagnosis, treatment, and follow-up for patients/clients in the home setting. Responsible to implement the nursing process, the coordination and continuum of care, patient/client assessment, patient/client education, triage, and various other nursing interventions. Functions within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in state of practice and licensure. The Professional Nursing Practice recognizes the Scope and Standards of Practice and the Code of Ethics for Nurses with Interpretive Statements as published by the American Nurses Association as the foundation of nursing care delivery and professional conduct. Assures physicians’ orders are processed and physicians are notified in a timely manner as needed while observing and documenting observations and care given to residents. Directs and assists with resident, family, and significant other education in conjunction with care team. Seeks to evaluate outcomes based on an integration among established clinical, financial and utilization data. Functions in structured and unstructured health care settings described as a geographic and/or situational environment that may not have established policies and procedures. Utilizes independent nursing judgment when integrating health care. Has knowledge of and utilizes appropriate age-specific structured care methodologies, such as protocols/integrated clinical pathways/guidelines/standards of care relating to the overall health care needs of the residents. Interviews potential residents and assesses residents in compliance with regulations, policy and procedure. Uses assessment information for admission and continued stay in the long term care community. Ensures all residents meet assisted living qualifications as identified in the occupancy agreement, if applicable. Establishes a service plan for each resident and evaluates on an on-going basis. Able to work with growth and development needs of client populations in clinical area. May work with clients in all age groups. Ability to communicate with residents, family members and others on the health care team. Coordinates, facilitates, and negotiates with others. Monitors, evaluates and trends resident responses utilizing structured care methodologies. Fiscally responsible. Controls variances and duplication. Must demonstrate independence, assertiveness and critical thinking when working with residents and co-workers. Ability to document clearly. Ability to solve problems. Self-motivated. Works with little direction. Ability to incorporate teaching/learning principles and adapt teaching to age group. Ability to facilitate groups and implement projects when necessary. Must be able to establish priorities, have strong flexibility and organizational skills. Must be knowledgeable about reimbursement for services provided. Will be required to work primarily day hours, scheduled weekends, and occasional evenings. May be exposed to communicable or infectious disease, hazardous materials and injury from performance of assigned duties. Is subject to multiple sensory and environmental stressors. Qualifications Bachelors degree in nursing preferred. Graduate from a nationally accredited nursing program preferred, including, but not limited to, Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA). Minimum of one to two years of professional nursing experience in a clinic care setting preferred. In-depth knowledge of specialty area preferred. Currently holds an unencumbered multistate RN license with the State Board of Nursing. Obtains and subsequently maintains required department specific competencies and certifications.
